Established in 2009 by Dr. Chad Fellows, we have been committed to providing eye care services inside Costco six days per week.

Our team works diligently behind the scenes to assist you to schedule timely appointments, answer questions, and make your experience with us flow smoothly from start to finish. We want to help you achieve your best vision.

We no longer bill any vision or medical insurance, but we are happy to help you with exams for glasses, contacts, or medical evaluations. We are a fee-for-service clinic.

Our Mission

We strive to provide exceptional vision care to the Colchester, VT community, conveniently located inside Costco.

Although not pediatric specialists, we see patients from 4 years old and older. We recommend younger patients see a pediatric eye doctor.

The vision of our success is the success of your vision. We want you to have a positive experience from the moment you connect with us online, by phone, or in office until leaving our office after your eye exam. We want you to see life clearly with glasses, contacts, or facilitating vision-correction surgery.

A Heart for Service

The doctors and staff at EyeFellows are deeply committed to making the patient experience smooth from start to finish. We want to respond quickly to patient appointment requests, facilitate referrals to other specialists, and assist in solving patient vision challenges.

We stay late, work weekends, and after hours in a coordinated effort to provide a positive experience for your eye exam. We appreciate patients and hope that shows as you trust us with your vision.

Dr Fellows has enjoyed providing essential eye care on international missions to Kazakhstan, Panama, the Dominican Republic, and multiple regions in Mexico, including Nochistlan and Zacatecas.

Community Outreach & Collaboration

Dr Fellows previously networked with community organizations locally and internationally to bridge the gap in healthcare accessibility.

This included providing eye exams for athletes at the 2002 Olympics in Salt Lake City Utah, and extensive service with Onesight, where he provided vision care to students in Utah schools and individuals within the homeless community.

We do not bill any vision or medical insurance and do not provide insurance billing, coding, or consulting services. We are a fee-for-service office providing visual and ocular examinations.

Meet the Doctors

Dr. Chad Fellows, O.D.

Dr. Chad Fellows is a graduate of Pacific University, OR (Class of 2000),He has practiced in Hawaii, Oregon, Utah, and Vermont in many different clinic settings which has proven valuable for patient engagement. He is currently a Lt Col in Vermont Air National Guard providing mission ready eye exams to F-35 pilots. Dr Fellows has enjoyed multiple international humanitarian eye care trips and previously volunteered for Olympic athletes, students in need, and homeless. Outside optometry Dr Fellows loves meaningful time with his wife of 30+ years, 4 children and 1 grandson. He loves travel, beaches, and experiencing new foods, restaurants, and activities.

Dr. Jeff Weintraub, O.D.

Dr. Jeffrey Weintraub received his Bachelor of Science degree from Northeastern University in Boston, MA in 1985. He earned his Doctor of Optometry degree in 1992, from New England College of Optometry in Boston, MA. Dr. Weintraub has practiced in Massachusetts and New York, and is a Vermont resident since 1996.

Dr. Suraj Afshar, O.D.

Suraj Afshar, O.D., received his Doctor of Optometry from the New England College of Optometry. He studied pre-medicine at the University of Hartford in Conneticut. After a great childhood experience with his family optometrist, Dr. Afshar knew what career he wanted to pursue. In his own words, Dr. Afshar says "I am grateful and enjoy being an optometrist because it provides the opportunity to give my patients the best vision possible while I learn more about their lives and families." In his professional career, Dr. Afshar has worked in corporate, private, and surgical optometry settings. His decade of experience includes ocular disease, complex contact lens fittings, dry eye, and general family vision care. He also specializes in Ortho-Keratology, allowing patients to be glasses and contact lenses free during the day while enjoying perfect vision. He is happy to accept new patients and has his therapeutic pharmaceutical agent (TPA) license to treat eye disease, infection, and injury. When he is not treating patients, Dr. Afshar likes spending time with his family and enjoys sports and outdoor activities
